Why have we chosen this as a priority area of focus?
We know that access to services is an issue at the moment and many people are waiting too long for the care they need. We want pathways that are quick, simple and efficient. People should be able to access the right care, at the right time that is delivered safely with no avoidable harm caused. At the same time, people should have access to care in a way that suits them with our pathways being adaptable and focused on the individual.
What do we want to see 10 years from now?
- Pathways are centred around individuals and systems are joined up.
- It is easy for members of the public to identify how to get the help they need.
- Best practice pathways are in place, ensuring high quality and safe care.
- All those who wish to use digital systems can or have an appropriate alternative mechanism that is equitable.
How will we know if we have achieved that?
- Unplanned demand has been reduced and/or shifted to planned care.
- Both emergency and planned care are able to be delivered in a high quality and timely way.
- Improvements in patient and staff experience feedback.
