Why have we chosen this as a priority area of focus?

We will work with partners to ensure that the remarkable community and neighbourhood work to support each other during the pandemic continues in the future. People looking out for and caring for each other create resilient, healthy communities. We will be an active and committed partner in sustainable community development and planning to integrate the work already being done to support communities to be adaptable, resilient and thriving .

We want our health and social care system to:

  • Listen to what is important to people and works with them to build the best possible health and social care support.
  • Bring together health and social care to support communities to take an active role in their health and to live as well as possible with their health conditions.
  • Work with communities and partners to shape places for health, places where we learn, live and work.
  • Empower people to feel in control, valued, motivated and supported.

 

What do we want to see 10 years from now?

  • An ongoing dialogue between us and our communities, with work led by the expressed needs of communities.
  • Communities are empowered by involvement in engagement work, enabling community-led action.
  • Connection with marginalised and seldom-heard communities is continually reinforced.
  • Communities are acknowledged as an integral part of our system – leading and participating.

 

How will we know if we have achieved that?

  • Different aspects of engagement with communities are collated and shared so that there is broad understanding of the themes and issues arising and the evolving needs of the population.
  • Decision-making is undertaken with communities as equal partners and some is led by communities.
  • Wellbeing in communities is improved as a result of their ability to connect over collective action.
