Preparing for Discharge

The nurses and medical team will discuss with you the date we are planning your discharge. Estimated discharge dates are a date that you and the ward team will be working towards. You will be able to discuss this with your family/carers so any support you require to progress can be put into place. Please be aware that this date may change if your medical condition changes or further tests/investigations are required. Any changes will always be discussed with you.

Discharge Day

When it is decided that you are ready to be discharged you need to be prepared to leave hospital. Your home or the ongoing place of care arranged will be the most suitable environment to meet your needs at this time. On your planned discharge date, unless there are any last-minute changes, we will support you to leave the ward by 10.30 am. If your relatives are unable to arrange transport at this time or you are awaiting discharge medications, you will be transferred to the discharge lounge until your medications are ready.

If for any medical reason you need transport this needs to be identified 48 hours prior to your discharge date. Hospital transport has a strict policy for who they can take home. If there is no medical requirement for the transport your request may be declined, and you will have to make your own arrangements. Please be aware that if patient transport is provided there is only space for one medium sized bag to travel with you and no definite time can be given of when you will be collected during the day.
